What Does Bone Loss Look Like In The Mouth?
Noticing Little Black Gaps Between Your Teeth? It Could Be Bone Loss
Have you ever looked in the mirror and noticed small, dark gaps between your teeth near the gums? These are called black triangles, and they’re more common than most people realize. While they may seem like just a cosmetic concern, black triangles are often a sign of something deeper—bone loss around the teeth.
At District Dental Surgery, with locations in Washington, DC and Bethesda, MD, we help patients understand what these changes mean and how to treat them the right way.
What Are Black Triangles?
Black triangles are the small, open spaces that can appear between teeth, usually near the gumline. In a healthy smile, this space is filled with gum tissue (called the papilla), creating a natural, full appearance. When that tissue is no longer supported, it recedes—leaving behind the visible gap you see as a black triangle.
What Causes Black Triangles?
The most common cause of black triangles is bone loss. Your gum tissue sits on top of the underlying bone. When bone is lost—often due to periodontal (gum) disease—the gum tissue follows, pulling away and creating space between the teeth.
Other contributing factors can include:
- Gum disease (periodontitis)
- Natural aging
- Orthodontic tooth movement
- Aggressive brushing or trauma
- Tooth shape (triangular-shaped teeth are more prone)
But in many cases, the root issue is the same: loss of bone support around the teeth.
Can Bone Loss Be Reversed?
This is one of the most common questions we hear. In most cases, bone loss cannot be fully reversed. However, it can be stabilized and managed with the right periodontal treatment. The goal is to stop the progression, protect the remaining bone, and create a healthier environment for your gums.
What are Treatments for Bone Loss (Black Triangle)?
Treatment depends on the severity of bone loss, the position of your teeth, and your overall gum health. There is no one-size-fits-all solution, which is why a proper evaluation is important.
- Periodontal Treatment (Foundation First)
- Stabilizing the foundation is always the first step.
- Before addressing the appearance of black triangles, we focus on treating the underlying issue—gum disease and bone loss.
- This may include:
- Scaling and root planing (deep cleaning)
- Periodontal maintenance
- Infection control
- Gum Grafting (When Appropriate)
- In certain cases, we can thicken and strengthen the gum tissue using a connective tissue graft.
- While this does not always fully “fill in” black triangles, it can:
- Improve gum health
- Provide better tissue support
- Help prevent further recession and bone loss
- Orthodontic Treatment
- In some patients, adjusting the position of the teeth can help reduce or minimize black triangles by changing how the teeth contact each other.
- Restorative Options (Selective Cases)
- Treatments like composite bonding may sometimes be considered, but they are not always recommended, especially when bone loss is present. Adding material to the teeth can make areas harder to clean and may increase the risk of plaque buildup if not carefully planned.
